• $200 - Psychiatric Follow-up Appointment

• $200 - Tele-Psychiatric Follow-up Appointment

• $100 - Late Cancellation / No Show (Less than 24 hours notice)

• $25 -   Phone Consultation (5 - 10 Minutes)

• $50 -   Phone Consultation (11 - 20 Minutes)

• $75 -   Phone Consultation (21 - 30 Minutes)

• $125 - General Letters, FMLA & Disability Paperwork

• $25 -   Point-of-Care Urine Drug Screen
